Equipment failure

According to a report in The Guardian newspaper (9 June 2025), NHS England has confirmed that since 2022, failures in equipment such as defibrillators in the NHS in England, have been responsible for the deaths of 87 patients, with a further 68 suffering serious harm and permanent damage or reduced life expectancy as a result of failed or outdated equipment. In addition, 522 patients have been moderately harmed in such a way as to limit their independence for up to 6 months.
