The Thirlwall Inquiry Report and Recommendations

This Report was published on 22 September 2026 by NHS England at a time acknowledged to be "a sobering moment for all working in the NHS". ¹ Its origins are the devastating failures of the NHS, at both the individual and collective level, to act in good time to safeguard newborn babies. It is stressed in the Report that reform is not just a matter for Trusts with neonatal services, but a matter for every single Trust.
