Wrong nebulizer caused baby death

A coroner has returned a verdict of accidental death contributed to by neglect in the case of a four-month-old baby that died after the wrong nebulizer tube was used. Instead of helping him breathe, it blocked his throat. The baby was recovering from a tracheotomy when the nebulizer tube was fitted. Nurses had failed to check whether the nebulizer was the correct one.
