Investigations still running suggest faulty equipment rather than sabotage was the likely cause of five incidents (one fatal)
of blocked oxygen tubes at hospitals earlier this year. Whether the fault will be found as intrinsic to the equipment, or
due to lack of proper use, is still uncertain.
