HSE criticises track faults, and itself

The third interim report from the Health & Safety Executive (HSE), just prior to the opening of Lord Cullen’s inquiry into the 31-death disaster, suggests track fault as a possible factor. A track joint ‘with significant misalignment between the rails’, near to the Automatic Warning System, could have led to vibration triggering an ‘all clear’ bell in the driver’s cabin.
