Employer fails to provide a safe system of work

The claimant was trying to lift a cone shaped casting that had fallen from a tray and become wedged. Whilst it was normal working practice for the crane to be used if fallen castings were not jammed, the claimant tried to use the crane to remove the jammed casting. He attached a hook from the crane operated by a hand held control to the narrow end of the casting.
